EPFO’s Dictum: Display Form 5A Extract at Establishment Entry Point by 21 Oct 2025
Central PF Commissioner—under Para 78(3) of the EPF Scheme, 1952—directs employers to prominently display the
Form 5A Extract at the entrance, website, and mobile app. Non-compliance may invite action under the EPF & MP Act, 1952.
Deadline: 21 October 2025 (15 days’ window)
Who must act: All covered establishments (ACC(HQ)/Zonal offices to ensure outreach)
Where to display: Entrance (physical board) + Official website + Mobile application
Form 5A (Extract): Current Functionality & Purpose
Employers must file/upload Form 5A Extract online detailing ownership & control—branches, owners/occupiers,
directors/partners, managers, and persons exercising ultimate control. It’s a one-time filing and must be
re-uploaded on change. Digital signature (DSA) is required on the EPFO portal.
Display extract must include (at minimum):
- EPF Code
- Registered Name
- Date of Coverage
- No. of Branches & Address of Primary Branch
- EPFO Regional Office
Form 5 vs Form 10 vs IW-1 — What’s Live?
- Form 5 & Form 10: Discontinued after UAN-based onboarding captured new joinee data on the portal.
- Form IW-1: Active for International Workers; monthly upload by the 15th (file NIL return if none).
15-Day Compliance Checklist (Practical)
- Verify latest Form 5A details on EPFO portal; update & re-upload if any change.
- Prepare a clean, readable extract (EPF Code, Name, Coverage Date, Branch count & primary address, Regional Office).
- Design a physical display board for the main entry (weather-proof, high-contrast type).
- Publish the same extract on the company website (footer/compliance page).
- Update the mobile app (if any) with a Compliance/EPF 5A section.
- Document with photos/screenshots for audit trail; retain version history.
- Train reception/security on what the display is & how to guide inspectors.
- Monitor for ACC(HQ)/Zonal advisories & maintain correspondence copies.
Non-Compliance Risk
Failure to display the Form 5A extract within the timeline may trigger legal action under the EPF & MP Act, 1952. Keep proofs of display and digital publication handy for inspections.
