PROCEDURES AND PURPOSE EMPLOYEES STATE INSURANCE ACT

Sr	Record/Return/Forms	Purpose	Due Date of Submission
1	Form No:1	declaration and nomination form to be given to employee and the time of his joining if he is not insured.	Within 7 days after joining of new.
2	Form no:3 in duplicate	Return declaration forms, covering letters to be enclosed with declaration forms to be submitted to respective local	Within 10 days
3	Form no:6 in qradruplicate	six monthly return of contribution remitted. To be enclosed with challans for respective period i.e April to Setember and October to March	Within 42 days of the end of every contribution
4	Form no:7 (reg.32)	register of employees showing 6 monthly contribution reducted along with employer share and daily wages calculation.	To be shown to the inspector at the time of inspection
5	Form no:15(reg.66)	Accident book / Accident register	To be shown to the inspector at the time of inspection
6	Form no:16	Accident report to be filled in triplicate	Within 48 hours.
